Based at our North Adelaide Office, as our Aged Care Connector, you will deliver timely, accurate and relevant education & information to people seeking support to access Aged Care Services. You will engage with individuals, community groups and relevant aged care networks to educate and help navigate potential customers of the Aged Care system, assisting them to navigate its complexities and helping them choose the aged care service that best meets their needs.
You’ll also make an impact by:
- Assist customers to navigate the My Aged Care Gateway including supporting them to engage appropriate aged care assessments.
- Increase referrals to Helping Hand’s Home Care services as per set KPI’s through positive customer engagement and service promotion.
- Coordinate and arrange a program of speaking engagements to customers as well as the general community (service clubs, community groups and other organisations) in relation to accessing Aged Care Services.
- Assist with quality reporting and regulatory compliance record keeping where required.
- Ensure customers are central to all decisions and actions and that customer service standards are maintained.
What you’ll bring
To succeed in this role, you will have:
- The role requires a relevant tertiary qualification such as Health Science, Community Development/Services or Social Sciences.
- Demonstrated experience in assisting clients to navigate the Aged Care system
- Experience and demonstrable knowledge of the Aged Care Industry, Aged Care legislation including the Aged Care Standards, the My Aged Care system and contemporary models of aged care
- Proven public speaking skills and the ability to engage with and talk to all sectors of the community.
- Exceptional communication and interpersonal skills, including demonstrated experience in liaising with a wide range of internal and external stakeholders.
- Possess drive, initiative and the ability to positively adjust to changing requirements and situations.
- Demonstrated proficiencies in Microsoft suite e.g. Word, Excel, Outlook and other computer applications and an ability to learn new software programs.
- Strong understanding of service delivery frameworks and service modelling.
- WH&S knowledge and skills suitable for the position.
- First aid and CPR certificate or willingness to obtain
- Access to private motor vehicle and valid driver’s licence.
- Valid NDIS Worker Screening clearance or be willing to obtain one.
You’ll stand out from the crowd if you have some of the following:
- Extensive experience in assisting clients to navigate the Aged Care system to ensure they receive the best possible services that they desire.
- Knowledge of community networks, and resources and proven skill in utilising these for service development.
